Job Overview
We are seeking a dedicated and compassionate LVN to join our healthcare team. The ideal candidate will play a vital role in delivering high-quality, patient-centered care in the comfort of the patient’s home. As an LVN, you will be responsible for delivering nursing care (medication administration) under the supervision of registered nurses and physicians, ensuring that patients receive the best possible care. This role involves conducting home visits, administering medication, monitoring patient conditions.
Work Schedule: Twice a day, six days a week
Responsibilities
- Provide direct nursing care to patients in their homes according to individualized care plans.
- Administer injectable medications as prescribed.
- Monitor and assess patient conditions, vital signs, and responses to treatment, reporting changes to the supervising RN or physician.
- Educate patients and caregivers on disease management, medication adherence, and home safety.
- Maintain accurate and timely documentation of patient care.
- Collaborate with the interdisciplinary healthcare team to ensure comprehensive patient care.
Experience
- Active LVN license in Texas (Must be in good standing with the Texas Board of Nursing).
- Previous experience in home health, long-term care, or a similar setting preferred.
- Strong clinical skills with the ability to work independently.
- Excellent communication and interpersonal skills to engage with patients and families.
- Reliable transportation and willingness to travel to patient homes.
