The Clinical Support Services Lead/Registered Nurse Case Manager (RNCM) is responsible for the coordination of comprehensive healthcare and community-based services for a designated patient population. The Clinical Support Services Lead/RNCM develops, implements, coordinates, and evaluates health care plans in the clinic; collaborates with other health care providers, and community service agencies to coordinate patient care. The Clinical Support Services Lead/RNCM ensures adherence to the clinic's policies and nursing procedures, standards, and practices. In addition, the Clinical Support Services Lead/RNCM provides direct supervision for the clinical support services team consisting of the nurse case managers, medical record coordinators and referral coordinators
This is a full-time and in-office position located at the AvitaCare Atlanta Clinic (2140 Peachtree Rd.) General work hours are Mon - Fri: 8:00 AM to 5:00 PM.
About the role:
- Facilitates patient access to healthcare, and community resources; assists patient to develop natural resources and contact social support networks; develops resources as appropriate to meet patient needs.
- Works with patients to assess, develop, implement, monitor, and recommend modifications to comprehensive, cost-effective care plans, using a multidisciplinary approach.
- Coordinates, monitors, and evaluates provider services and care.
- Documents all patient encounters and contracts made on behalf of patients; documents nursing summaries of care.
- Evaluates and documents the progress of individual therapeutic programs, and recommends modifications as needed.
- Works closely with the interdisciplinary team to identify high-risk, complex patient cases; manage care including referral management, care planning, post discharge planning and coordinate transition of care post-hospital discharge.
- Serves as the primary point of contact for the following: (1) Department of Public Health and Health Departments for HIV/STI surveillance (2) Q-care plus for linkage to care for patients requesting transition to clinic environment. (3) HIV Patient self-referral portal on the AvitaCare Atlanta website and management of those patients linkage to care
- Conducts patient assessments for health coaching programs to reduce or eliminate high risk behaviors.
- Assists Clinical Lead with quality assurance monitoring with UDS/HEDIS and other quality metrics as dictated by the insurance providers. Works directly with FQHC as the primary quality point of contact for the practice.
- Partners closely with care management staff for coordination of social services needs.
- Performs all other duties as assigned.
About you:
- Registered Nurse (RN) with valid Georgia license
- BSN required
- MSN preferred
- Case Manager certification required
- Minimum of 2 years case management experience required.
- Minimum of 2 years clinical experience required.
- Active BLS certification required
- Must have working knowledge of the nursing process as well as in depth knowledge of therapeutic, physical, psychosocial, spiritual, cultural, and developmental needs for a select category of patients
- Good communication skills, both written and verbal
- Experience in HEDIS/UDS and quality improvement preferred
- eClinical Works experience is a plus
- Available to work in-office/onsite, Monday - Friday from 8:00 a.m. to 5:00 p.m.
- Must have excellent interpersonal and communication skills to promote optimal patient care
- Bilingual fluency is a plus but not required

Pay range: $91,000 - $111,000 per year, based on credentials and geographic location.
