POSITION PURPOSE:

Functions as Nurse Practitioner in an out-patient primary health care clinic, serving the low-income, working poor, homeless, and those individuals with barriers to health care of Douglas County. Under the direction of the Chief Medical Officer, the Nurse Practitioner assumes the responsibility of routine patient care including: assessment, diagnosis, and evaluation of injury and/or illness, and ordering of routine exams.  Utilizes skills in providing diagnosis and treatment of acute and chronic health problems, along with providing planning and problem solving with clinic staff and volunteers. Provides care consistent with policies and procedures of the organization.

 

ESSENTIAL FUNCTIONS:

  • Promotes a strong belief in Umpqua Community Health Center dba Aviva Health’s philosophy, purpose, mission, and ideals.
  • Performs history taking, physical examination, diagnosis, and therapeutic treatment planning within their scope of practice.
  • Demonstrates AVIVA HEALTH’s values and approaches all tasks with AVIVA HEALTH’s Mission, Vision, Values, and Customer Service statements as guidelines. Demonstrates care and compassion, respect, sharing, professionalism, confidentiality, collaboration, and teamwork. Takes personal responsibility.
  • Exhibits the ability to make effective decisions based on verifiable and measurable data/criteria.
  • Reviews and assesses the patient medical history, including health habits and disease history. Includes basic principles of healthy lifestyles and health risk reduction with patient encounter. Orders diagnostic testing as indicated.
  • Interprets results of diagnostic testing and plans appropriate follow-up with individual patients, with consideration given their physical, emotional, and financial resources.
  • Applies sound judgment and skill in emergency situations.
  • Demonstrates respect and sensitivity to cultural/social differences in interactions with others.
  • Demonstrates initiative and problem-solving skills, using sound judgment.
  • Meets and promotes organizational standards for excellence in service for patients, staff, and physicians.
  • Oversees activities of team to ensure consistent and coordinated patient care.
  • Maintains a complete and legible medical record, to include timely documentation of each patient visit.
  • Identifies resources available within the community to assist patients with their health care needs.
  • Plans and provides routine immunizations.
  • Prescribes selective medications in accordance with prescriptive privileges. Administers medications and injections accordingly. Responds to requests for medication refills in a timely manner.
  • Counsels patients regarding general physical and emotional concerns.
  • Provides patient education through handouts and discussion of patient concerns during office visits. Instructs patient/family regarding medication and treatment directions.
  • Provides consultation by telephone or in person to patients and medical personnel.
  • Identifies situations that are beyond the Nurse Practitioner’s scope of practice and refers them to appropriate provider.
  • Follows bloodborne pathogen exposure control plan guidelines and body substance precautions.
  • Attends all required staff meetings.
  • Continually updates knowledge through Continuing Medical Education. Meets all mandatory education requirements and participates in continuing education for professional growth.
  • Maintains professional affiliations.
  • Develops and maintains working relationships with the medical community.
  • Assumes additional responsibilities as needed, with little or no direction; shows initiative in assisting others within the department.
  • Works well independently and as a member of the team.
  • Willingness to work a flexible schedule when circumstances necessitate.

 

QUALIFICATIONS:

  • Graduate of an accredited school of nursing with advanced preparation as a Nurse Practitioner; Master’s degree preferred.
  • Current licensure in the state of Oregon as a registered nurse, with certification in the state of Oregon as a Nurse Practitioner with prescriptive privileges.
  • Holds current DEA license.
  • One-year professional nursing experience in a clinic setting.
  • Preference given to bilingual applicants. 


AGE SPECIFIC CRITERIA:

    • Has knowledge and skills to provide services within scope of practice which may include the following age specific groups:
      • Neonates (birth to 1 year)
      • Children (1 year to 12 years)
      • Adolescents (13 years to 17 years)
      • Adults (18 years to 64 years)
      • Geriatric (65 years and above)

Salary : $115,000 - $120,000
