POSITION PURPOSE:

The position of Psychiatric Mental Health Nurse Practitioner (PMHNP) shall be a core member of the interdisciplinary Patient Centered Primary Care Team. Provider will evaluate, diagnosis, prescribe and monitor medications and collaborate with patient’s primary care and/or behavioral health provider as appropriate.

 

ESSENTIAL FUNCTIONS:

  • Medically evaluates and assesses patient medical status.
  • Performs psychiatric evaluations including a differential diagnosis and develops and monitors treatment plans.
  • Prescribes psychiatric medications and collaborates with the behavioral health team and nursing staff to facilitate appropriate medication monitoring and adherence.
  • Works collaboratively as part of an integrated health care team with providers including, but not limited to, Physicians, Nurse Practitioners, Nurses, Mental Health counselors, Clinical Psychologist(s) and Psychiatrist(s).
  • Coordinates services with the patient’s primary care provider.
  • Provides crisis and suicide risk assessment and determines necessity for two-physician hold.
  • Completes timely and appropriate documentation in the electronic medical record.
  • Assists in the evaluation of utilization and impact of behavioral health services.
  • Adheres to Aviva Health’s compliance policy, departmental guidelines/policies and all applicable laws and regulations.
  • Complies with established departmental policies, procedures, and objectives.
  • Maintains patient confidentiality and complies with HIPAA regulations.
  • Attends a variety of meetings, conferences, seminars as required or directed.
  • Demonstrates use of Quality Improvement in daily operations.
  • Complies with all health and safety regulations and requirements.
  • Respects diverse views and approaches, and contributes in maintaining an environment of professionalism, tolerance, civility, and acceptance toward all employees, patients, and visitors.
  • Performs other similar and related duties as required or directed.

QUALIFICATIONS:

  • Nurse Practitioner or Family Nurse Practitioner with specialized Certification in Psychiatry (i.e. PMHNP). Licensed to practice medicine in the State of Oregon.  Requires DEA registration.  Current BLS certification.
  • Knowledge in the theory, principles, and practices of psychiatry; the assessment, diagnosis, and treatment of psychiatric conditions; ability to assess and treat patients with dual diagnosis presentation; knowledge of behavioral intervention, management, and support strategies; sound case management; knowledge of community resources; knowledge and use of evidence-based treatments.
  • Skill in communications, both written and verbal, that promotes professional development and ethical practices and meet agency standards. Ability to collaborate as part of a multidisciplinary treatment team, to promote improved health in patients.
  • Ability to clearly and professionally document in the EMR.
  • Must be able to interact with a variety of individuals at various levels under stressful circumstances while exercising sound judgement, tact, and diplomacy.
  • Experience using electronic medical record, Microsoft Office products, and computers.

Douglas County stretches from the Pacific Ocean to the Cascade Range and world-class outdoor activities are within a couple of hours or less of the county seat, Roseburg. The coast is roughly an hour west, dozens of waterfalls and hiking trails are minutes away and the rivers, lakes, reservoirs, and streams are some of the best fisheries in the Pacific Northwest.
