Your Role
As a Street Outreach Navigator, you’ll be on the frontlines—offering direct support to individuals experiencing chronic unsheltered homelessness, many of whom may also be navigating opioid use or mental health challenges. You’ll build trusting relationships in parks, encampments, and other outdoor spaces, working toward connecting people to shelter, services, and permanent housing. You’ll be an advocate, connector, and consistent presence—providing access to resources and planting seeds of hope.
This is a year-round, primarily outdoor, community-based position. The ideal candidate thrives in dynamic environments, values relationship-building, and brings cultural humility and resilience to their work.
Key Responsibilities Include
- Provide housing-focused street outreach to individuals living unsheltered in Minneapolis.
- Help individuals connect to shelter, housing, treatment, healthcare, and basic needs.
- Use harm reduction and progressive engagement to build trust and support individuals’ self-identified goals.
- Collaborate with internal teams, external partners, and the Coordinated Entry System.
- Maintain timely and accurate documentation using HMIS, Credible, and internal systems.
- Uphold confidentiality and HIPAA compliance standards.
- Participate in division-wide initiatives including racial equity, undoing racism, and service integration.
Qualifications
What You Offer
Required:
- High school diploma or GED.
- One or more years of experience working with individuals experiencing homelessness or related field.
- Comfort and commitment to working outdoors in all seasons.
- A deep belief in harm reduction, person-centered care, and housing as a human right.
- Strong communication and relationship-building skills.
- Valid driver’s license, insurance, and access to a personal vehicle.
Preferred:
- Lived experience with homelessness, mental health recovery, or substance use recovery.
- BA in Behavioral Science or related field.
- Experience in outreach or shelter settings.
- Familiarity with Housing First, trauma-informed care, and systems like HMIS or Credible.
