Responsibilities:
- Coordinate meetings with case managers, clinicians, community-based providers, and/or support workers;
- Maintains a caseload established with the Service Director in order to work closely with programs in assigned service areas;
- Assesses and evaluates adults to determine the appropriateness and need for case management services;
- Assists in the person's entry into service and monitors the needed services throughout service system;
- Provides coordination and comprehensive development of an Individual Treatment Plan (ITP) and monitors that plan as required by established rules;
- Follows State of Montana and A.W.A.R.E., Inc. rules, policies, procedures, and guidelines as they apply to this service;
- Assures that any person serviced by Adult Case Management is screened for and assisted in enrollment in the SDMI program;
Requirements
Talents, skills, and abilities:
- Bachelor’s Degree in Human Services or a related field.
- Minimum of one (1) year direct work experience.
- Valid Montana Driver’s License.
