AWARE, in partnership with Missoula Partnership Health, is looking for the right person to join the team as a lead family coordinator consultant.

Responsibilities:

  • Relate positively to families needing support services and have the ability to establish nurturing, positive relationships.
  • Observe and make accurate assessments of situations; possess critical decision-making skills.
  • Conduct client assessments in accordance with program objectives and applicable program rule.
  • Have the ability and skill to be able to develop the plan of care for each family coming into care.
  • Facilitate and motivate families to engage and connect to skills programs, including Parenting, Money Management, Job Skills, Health Issues and other topics encouraging personal and spiritual growth
  • Schedule regular meetings with clients using proper forms for documentation of each client’s progress.
  • Assess client progress and discuss all discharge plan decisions with Supervisor/Director.
  • Assure quality in the provision of all services.
  • Report any non-compliance issues with agency policies or expectations regarding facilities, programs and personnel to the Supervisor/Director.
  • Consult with Supervisor/Director on any issues or problems arising during service.
  • Represent the Agency at various profession or community organizations as requested, including interaction with County partners and SAMHSA grant committees.
  • Maintain caseload and service benchmark requirements as directed.
  • Participate in training opportunities to stay abreast of innovative ideas and tools available in the field.
  • Maintain flexible schedule to be available to meet with clients in evenings or weekends when necessary.
  • Perform such other duties as assigned by the Administrator of Statewide Programs.

Requirements


Talents, skills, and abilities:

  • Bachelor’s degree in social work, human services, or related filed. Other combinations of education and experience may be considered on a case-by-case basis.
  • Excellent interpersonal and communication skills.
  • Demonstrated ability to build and maintain relationships with diverse populations, including government officials, healthcare providers, and community organizations.
  • A conviction that people have the capacity to grow and change.
  • Possesses sensitivity to the cultural differences that are present among the organization’s service population and staff.
  • An ability to work in partnership with other members in a team approach.
  • An ability to demonstrate good verbal and written skills.
  • An ability to manage time, set priorities, to organize and to follow structured routines.
  • A commitment to uphold the ethical and professional standards of Social Work practice.
