AWARE's Program of Assertive Community Treatment (PACT) Psychiatric-Mental Health Nurse Practitioner (PMHNP) is responsible for providing leadership and guidance to PACT program staff concerning patient care. This includes providing direct psychiatric care to patients, as well as consultation and guidance to other members of the team. Duties will include, but are not limited to, performing psychiatric evaluations, contributing to team development of patient care plans, obtaining indicated diagnostic tests, prescribing medications and treatments, and providing ongoing psychiatric care. The PMHNP may also carry a small caseload of patients not enrolled in PACT services.
The PMHNP understands that AWARE is not attempting to institute a medical model in its services; rather, the PMHNP works within a team to help people live as independently as possible with dignity and respect, in keeping with the PACT model of care.

Cross-coverage for other psychiatric providers at AWARE may be included as a member of the psychiatric care team.
Responsibilities:
- Work within a treatment team to help provide coordinated mental/behavioral health care to patients.
- Complete comprehensive psychiatric evaluations for new clients; help with annual re-evaluations as needed
- Provider monthly (or more frequent) follow-up psychiatric visits with PACT patients
- Provide documentation of patient care in accordance with AWARE policy and professional standards, utilizing AWARE's electronic health record as outlined, trained, and assigned.
- Create psychiatric care plans and integrate with existing behavioral/psychotherapeutic plans/
- Order and interpret appropriate diagnostic tests.
- Prescribe appropriate psychiatric medications as indicated.
- Recognize physical health issues and refer for patients for care as necessary.
- Be available to teams as a resource regarding medications and treatments.
- Evaluate responses to treatments and medications.
- Coordinate care with other healthcare professionals both inside and outside of the organization.
- Act as an advocate for patients both within and outside of the organization.
- Provide education and support to staff, patients, and families on managing psychiatric illness and disability.
- Maintain licensure and certification required by AWARE, the State of Montana, and the federal government.
- Participates in peer/record review process and other quality improvement initiatives as assigned.
- Performs at or above productivity benchmarks identified by AWARE so that services are sustainable.
- Performs other duties as assigned.

Talents, skills, and abilities:
- Thorough knowledge and commensurate skills in providing psychiatric care to children, adolescents, and adults, including those with co-existing intellectual / developmental disabilities.
- Accurate and effective written and verbal communication skills, and comfort working as part of, and sharing decision-making with, a therapeutic team.
- Master's or Doctorate degree from an accredited nurse practitioner program;
- Must have APRN licensure in the state where employed or a compact party state and be board certified;
- Documented education and experience in providing psychiatric care as a nurse practitioner;
- Must have prescriptive authorization;
- Knowledge of computer-based data management programs and information systems, as well as medical records and point-of-interview technology; readiness to learn and adapt to new technology as it is adopted by AWARE, and to provide input to the selection and implementation of such technology
- Ability to communicate effectively, in verbal and written form, with partners at various levels: patients, family members, physicians, members of the treatment team, and representatives of the community;
- Sound understanding of all federal and state regulations including HIPAA and OSHA
