Pay range : $50-$60 per hour
Shift : Monday - Friday 8a-4:30p, No weekends, No major holidays, no -on-call required.
Job Description :
Required Exp: 2 years post MSW Acute Care experience, Medical Social Work, Case Management, Brief Treatment, ICU, Critical Care, Chronic Illness, Pain Management, Complex Care.
Preferred Exp: Housing Experience, Substance Use Disorder, Grief and Bereavement , Cultural Responsiveness, Crisis Intervention.
Required Certs: Licensed at the Masters Level with Masters in Social Work degree, BLS
Dept Details:
1. Conduct comprehensive psychosocial assessments to identify patient and family needs, strengths, and resources in both inpatient and outpatient settings.
2. Develop and implement individualized care plans, including psychosocial diagnoses and tailored interventions, to support patients' medical and psychosocial goals.
3. Provide counseling, crisis intervention, and emotional support to patients and families dealing with the acute and chronic impacts of SCD, including pain management, treatment adherence, and care transitions.
4. Round with interdisciplinary inpatient teams to ensure seamless integration of psychosocial support into patient care plans.
5. See patients in outpatient clinics, providing ongoing support, education, and advocacy to address the long-term management of SCD.
6. Facilitate education for patients and families to enhance understanding of SCD, promote self-advocacy, and foster resilience in managing their condition.
7. Collaborate with physicians, nurses, case managers, and other interdisciplinary team members across inpatient and outpatient settings to deliver holistic care.
8. Serve as a liaison with external agencies, such as social service organizations, schools, and community resources, to advocate for patients and secure access to necessary supports.
9. Consult with legal and governmental entities, including courts and public assistance programs, to address systemic issues impacting patient care.
10. Provide mentorship, education, and coaching to patients, families, and team members to improve care outcomes and patient satisfaction.
11. Participate in program development, quality improvement initiatives, and efforts to enhance the continuum of care for patients with SCD.
