Licensed Clinical Social Worker (Care Navigation)
Summary
The Licensed Clinical Social Worker (LCSW) is a skilled professional who offers essential services, support,
and resources to individuals and families as part of our Care Navigation team. Working closely with the Diagnostic team, the LCSW plays a key role in delivering top-quality diagnostic care. This position may involve seeing clients both in person and through telehealth sessions. The LCSW will report directly to the Care Navigation Program Manager.
Your Role at Axis For Autism:
- Provides a range of social work interventions that include, case management, counseling, crisis intervention, and advocacy in assisting patients and families reach their goals.
- Assesses the psychosocial status of patients related to the patient or family’s needs and environment.
- Reviews diagnostic recommendations and provides support for community resources which most effectively meet the identified patient and family needs
- Facilitation of interdisciplinary communication as it relates to patients and families experiencing complex psychosocial issues.
- Educates patients, families, and AXIS team members regarding pertinent patient care issues, and population specific information.
- Serves as patient advocate and liaison with physicians, families and community to ensure continuity of care.
- Identifies and participates in Quality Improvement, research and program evaluation initiatives that focus on improving health care services.
- Maintains clinical records on all patients following organization and state board requirements.
- Completes timely and well-written clinical and progress notes that follow clinical standards.
- Provides information and referral services for patients and families/caregivers regarding diagnostic recommendations, SDOH needs, and other referrals requested by family.
- Maintains collaborative relationships with AXIS clinicians and employees to support patient care.
- Facilitation of interdisciplinary communication as it relates to patients and families experiencing complex psychosocial issues.
- Educates patients, families, and AXIS team members regarding pertinent patient care issues, and population specific information.
- Maintains and develops contracts with public and private agencies as resources for patients and organization personnel.
- Complies with the legal and regulatory law including mandated reporting requirements.
- Serves as a supervisor for social work graduate training as appropriate to supervisee’s skills when needed.
- Other duties as delegated by the Care Navigation Program Manager.
Who You Are:
- Active Licensed Clinical Social Worker is required
- A master’s degree in social work is required
- Demonstrates good verbal and written communication, and organization skills
- Ability to analyze, intervene and evaluate a range of psychosocial theory and methods in practice with clients; and demonstrated knowledge of community and economic resources.
- Ability to serve culturally and linguistically diverse families in low and high social economic status areas.
