As a Home Health Aide, Axzons Home Healthcare expects that patients be provided with assistance in daily living and personal care services in their home in accordance with an established care plan. Axzons is looking for Home Health Aides / Personal Care Assistants in the Ronkonkoma New York Area. Part Time and Full Time Day Shifts only.
Responsibilities :
- Provide health care services in patients residences
- Perform domestic and household tasks
- Accompany patients to doctors office or to hospital
- Administer simple prescribed medications
- Assist with clients personal care activities
- Maintain patients care records and document provided services
- Assist patients with mobility and physical therapies / exercises
- Provide companionship and basic emotional or psychological support
