Job Title : Long-Term Acute Care (LTAC) Registered Nurse (RN)
Job Summary :
A Long-Term Acute Care (LTAC) Registered Nurse (RN) provides specialized, extended care to patients who require prolonged recovery periods due to complex, chronic, or severe medical conditions. These patients often have serious health issues that require intensive monitoring, specialized treatments, and extended stays in healthcare facilities. LTAC RNs work closely with interdisciplinary teams to ensure comprehensive, patient-centered care and promote optimal outcomes for long-term recovery.
Key Responsibilities :
- Provide direct nursing care to patients with complex medical conditions requiring extended recovery, including ventilator care, wound management, and intensive monitoring.
- Administer medications, treatments, and interventions according to physician orders and clinical protocols.
- Monitor and assess patients' conditions, including vital signs, lab values, and response to treatments, adjusting care as necessary.
- Collaborate with physicians, therapists, and multidisciplinary teams to develop and implement individualized care plans.
- Educate patients and their families about their conditions, treatment plans, and strategies for continued care after discharge.
- Document patient assessments, care provided, and outcomes accurately and in compliance with regulatory standards.
- Respond to and manage acute or chronic complications, such as respiratory distress, infections, and other health issues.
- Supervise Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs) in delivering care and ensuring high standards of patient safety and quality.
- Maintain infection control and safety protocols to minimize risks to patients.
- Participate in quality improvement initiatives, continuing education, and professional development to stay current on best practices in long-term care.
- Advocate for patients and coordinate care with external services, including home care and rehabilitation facilities.
Qualifications :
Valid Registered Nurse (RN) license in the state of practice.
Basic Life Support (BLS) certification required.
