Job Summary:
This is an exciting opportunity for a highly skilled and motivated LMSW to work with patients and families to provide comprehensive care and support.
Responsibilities:
- Develop and implement individualized plans of care to meet the unique needs of patients and caregivers
- Collaborate with internal and external teams to ensure seamless care coordination
- Document pertinent information to facilitate care coordination and communication
- Participate in care conferences and discharge planning activities
- Provide education and support to patients and caregivers to promote self-care and independence
- Advocate for patients' rights and interests
- Maintain accurate and timely documentation of patient care and services
Requirements:
- Master's degree in Social Work from an accredited program
- Current state licensure as a Master's level Social Worker
- At least one year of experience in social work or a related field
