Citadel Health (citadelhealth.com) a subsidiary of Aylo Health is hiring! We seek an experienced Population Health Manager to join our population health initiatives and drive improvements in health outcomes across our patient population. The ideal candidate will combine clinical expertise with data analytics skills to develop and implement effective population health strategies.
Required Qualifications:
- Experience working in a community practice or medical setting is strongly preferred
- Minimum 3 years’ experience in population health management or a similar role
- Strong background in healthcare analytics and quality improvement
- Experience with value-based care models and risk stratification
- Experience with eClinicalWorks (ECW) or similar electronic health record systems
- Strong time-management skills and the ability to operate independently
- Proven planning and project management skills
- Proficient in Microsoft Excel and Microsoft Office
- Open to travel in the state of Georgia
Key Responsibilities:
- Lead care management initiatives and programs
- Analyze population health data to identify trends and opportunities
- Create and monitor quality metrics and performance indicators
- Collaborate with healthcare providers and community partners
- Develop and implement population health strategies
- Design and implement interventions to improve health outcomes
- Report on program effectiveness and ROI
Location: Remote, preferred in middle or central Georgia. Possible for Northern Atlanta area.
