**Position Summary:**
The Client Care Coordinator at AZ Tender Hands plays a vital role in managing client care and expanding our services. This position involves handling new client intakes, seeking and processing referrals, conducting home visits, maintaining accurate client data, sourcing resources for clients, and participating in public and outreach events to promote the growth of AZ Tender Hands.
**Key Responsibilities:**
1. **Client Intake & Referrals:**
- Oversee the intake process for new clients.
- Actively seek and accept new referrals.
- Utilize the portal/email or communicate with case managers to request client information and referrals.
- Follow the established procedures for new referrals and client intake.
2. **Referral Processing:**
- Handle referrals in a timely manner.
- Introduce potential Direct Care Workers (DCWs) to clients ensuring compatibility.
- Coordinate the training of DCWs to meet specific client needs.
3. **Client Care Monitoring:**
- Monitor the services provided to ensure that all client needs are met.
- Maintain regular communication with the team to update on client status and care.
4. **Reporting & Outreach:**
- Compile new referral information and generate reports on client acquisition.
- Participate in public events and outreach programs to increase AZ Tender Hands’ client base and ensure company growth.
5. **Filing and Office Organization:**
- Keep the office environment neat and organized.
- Perform filing duties on a daily and weekly basis to ensure all client information is up-to-date and easily accessible.
**Qualifications:**
- Experience in client care coordination, preferably within a healthcare setting.
- Strong interpersonal and communication skills.
- Ability to work independently and handle multiple tasks.
- Proficient in data management and office software applications.
- Capable of conducting home visits and attending public events as needed.
- Knowledge of the local healthcare system and resources.
- Marketing Skills and People Person Highly Needed
- Strong Office and Computer Skills (Excel, Microsoft Office, and overall application knowledge.
