Position Overview: Under the general supervision of the Executive Leadership and Clinical Director, the Tailored Care Management Director will oversee and manage the delivery of tailored care services. This role ensures alignment with organizational goals, regulatory requirements, and evidence-based practices. The Director will lead a skilled team, foster staff development, and drive outstanding client outcomes across the department.
Key Responsibilities
Program Oversight and Management:
- Develop, implement, and monitor tailored care programs in compliance with organizational goals and regulatory standards.
- Ensure care services adhere to evidence-based practices and quality standards.
- Oversee person-centered care plans that address the unique needs of clients.
- Track department performance, including client outcomes, productivity, and compliance.
Leadership and Staff Supervision:
- Lead and supervise a multidisciplinary team, including clinical and administrative professionals.
- Conduct team meetings, performance evaluations, and supervision sessions.
- Promote professional development and training opportunities for team members.
- Address staff concerns and foster a collaborative, supportive environment.
Client Care and Advocacy:
- Collaborate with team members to ensure individualized, comprehensive client care.
- Serve as a liaison between clients, families, staff, and external stakeholders.
- Respond to escalated client concerns and ensure satisfactory resolutions.
Compliance and Quality Assurance:
- Ensure department compliance with applicable policies and regulations.
- Oversee client record audits and documentation reviews.
- Implement quality assurance initiatives to enhance service delivery and outcomes.
- Stay informed on industry trends and regulatory updates.
Community and Stakeholder Engagement:
- Develop and maintain relationships with community partners, Managed Care Organizations (MCOs), and other stakeholders.
- Represent the organization at meetings, conferences, and networking events.
- Lead outreach efforts to improve service accessibility and visibility.
Qualifications:
Education: Master’s degree in Social Work, Psychology, Counseling, or a related human services field.
Licensure/Certification: Provisional or Full licensure required.
Experience:
- 3-5 years of leadership experience in human services or healthcare.
- Proven success in managing client-centered care programs and multidisciplinary teams.
- Knowledge of person-centered planning, tailored care approaches, and compliance standards.
Work Environment:
- Flexible work environment with office-based, community-based, and remote settings.
- Occasional travel to meetings, conferences, and service sites.
