Requirements :
Board Certified - REQUIRED if Board Eligible(ABIM ONLY)
Active WA license or IMLC(LOQ in hand / must upload during name clears) - REQUIRED
Completed Endocrinology Fellowship - REQUIRED
Malpractice - Clean Preferred
Availability at time of name clear(Required 30 days of availability from providers start date) - REQUIRED
ACLS / BLS / DEA - REQUIRED
