What are the responsibilities and job description for the Nurse Home Visitor position at Baby Bear Hugs?
Purpose
The Nurse-Family Partnership (NFP) Nurse Home Visitor is responsible for providing comprehensive nursing services to women and their families eligible for the NFP Program. Nurse Home Visitors (NHV) are responsible for maintaining the highest standards in clinical nursing practice and adherence to the Nurse-Family Partnership model, policies, procedures, Visit-to-Visit Guidelines, and standards of Nurse-Family Partnership and Baby Bear Hugs. The nurse home visitor reports to the Nurse-Family Partnership nurse supervisor.
Basic Job Functions
• Provides home visits to women and their families eligible for the NFP Program.
• Adheres to nursing process and the NFP model of home visitation.
• Carries a caseload of 25 or more clients per 1 FTE (full time position).
