Job Overview
The Care Manager will be responsible for ensuring the highest level of client care and operational efficiency in our Tempe office. This role involves conducting comprehensive assessments, managing care plans, overseeing staff, ensuring compliance with regulations, and driving sales and referral growth.
Key Responsibilities
- Conduct initial assessments and periodic reassessments in diverse care settings, including hospitals, homes, and facilities.
- Develop and implement comprehensive care plans tailored to each client's needs.
- Regularly review and update care plans to align with changes in client health and requirements.
Caregiver & Client Management:
- Oversee the recruitment, engagement, retention, and professional development of caregivers.
- Conduct annual performance reviews and facilitate conflict resolution to maintain a cohesive and effective care team.
- Serve as the primary contact for clients and their families to ensure satisfaction with care services.
Compliance & Regulatory Adherence:
- Ensure office operations comply with local licensure laws, ALTCS, VA, and Medicaid regulations.
- Maintain state survey readiness and ensure proper documentation practices are observed.
- Stay informed about changes in healthcare regulations and adapt strategies as required.
Sales & Referral Growth:
- Build and nurture relationships with ALTCS, VA in Arizona, and private pay referral sources.
- Develop strategic initiatives to drive new client referrals and business growth.
Operational Oversight:
- Ensure the smooth operation of office activities, including efficient scheduling of caregivers.
- Maintain accurate, timely documentation and reporting for all client interactions and office operations.
Qualifications & Experience:
- Proven experience in home care management is essential.
- Demonstrated strong leadership and problem-solving skills.
- Ability to build and sustain referral relationships and drive business development.
- Highly adaptable and comfortable working in a fast-paced, high-growth environment.
- Possession of a reliable vehicle and a clean driving record is required.
