Job Summary
We are seeking a full-time Physical Therapist to join our team. The ideal candidate will be responsible for planning, providing, and coordinating all aspects of direct patient care in accordance with established clinic and departmental policies and procedures.
Essential Duties and Responsibilities:
- Provide high-quality patient care and assessment in accordance with established practice standards and clinic/departmental policies in a safe environment.
- Perform patient evaluation, summarize objective clinical findings, and develop an accurate individualized plan of care.
- Provide treatment in accordance with physician's orders and established plan of care.
- Evaluate effectiveness of treatment plan and revise plan of care as indicated.
- Communicate/collaborate with patient, family, caregivers, significant other, and other members of the healthcare team to promote maximum benefit of care.
- Assess educational needs of the patient, family, caregiver, or significant other and other members of healthcare team to promote maximum benefit of care.
- Demonstrate appropriate knowledge and correct application of clinical techniques, procedures, and programs for clinical setting and patient/client populations served.
- Plan and make appropriate discharge plans.
- Assess the risks for safety and implement appropriate precautions.
- Comply with appropriate and approved security and safety standards.
- Utilize appropriate leadership skills in delegating, organizing, and educating coworkers and staff.
- Participate in and contribute to quality improvement process.
- Makes effective use of patient schedule and time by achieving various efficiency standards such as skilled units per visit, visits per day, and evaluations plus two-skilled units.
- Complete timely, competent, and compliant documentation using AgileRPM.
- Travel to other local clinics as assigned.
Qualifications:
- Master's or Doctorate degree in Physical Therapy.
- Current Physical Therapist license, registration, and/or certification as per state regulations.
