Position Summary:
The Director of Care Coordination is responsible for leading a team of clinical professionals at both Backhus and Windham Hospitals in the execution of exemplary cross-continuum care coordination and has full accountability and oversight for the overall operations of the Care Coordination department. The Director is responsible for the development and implementation of the case management process and promoting quality patient care outcomes while supporting appropriate utilization/resource management along the continuum of care. The Director will facilitate the multidisciplinary case management process by coordinating the development and implementation of tools and systems for effective, efficient case management to ensure appropriate care coordination and care transition handoffs for all patients.
Key Accountabilities:
Implements the inpatient care coordination and social work strategies based on developed objectives and provides leadership in the continuing development and refinement of care coordination and utilization review activities, including but not limited to Coordination of Care, Centers for Medicare & Medicaid Services (CMS) Conditions of Participation, Discharge planning, Social Services, Readmission prevention, Length of stay, excess days and avoidable days, Value Based Care partnerships, Care Manager's roles in Multidisciplinary Rounds, Care transition and Colleague/ customer experience
Contributes to development of performance standards, appraises performance and works to assess, maintain, and improve competency in CMS regulatory processes and safe transitions of care
Determines, provides and/or implements proactive local and corporate strategies and methods necessary to improve operations and outcomes for a high-performance culture, including operationalization of system standard work
Cultivates and maintains intra departmental and medical staff relationships relative to care coordination objectives
Manages interface with third party review and external agencies relative to compliance with regulatory and contract requirements
Coordinates, mentors and supervises the hiring, orienting, evaluating, continuing education and duties of the clinical RN case coordinators, case manager assistants, and social workers
Remains knowledgeable about the evolution of care coordination and social work on a regional and national level and recommends modifications as needed to maintain a state-of-the-art initiative
Analyzes outcome data to identify and make recommendations related to specific populations or diagnoses
Qualifications
Bachelor’s Degree in a health-related field required
Five plus years of clinical experience or acute care case management experience
Three plus years of healthcare leadership experience
Current license in good standing to practice as a Registered Nurse or related Health Care Professional in the State of Connecticut
Demonstrated knowledge of case management, discharge planning, transition of care, and transfer coordination; Medicare, Medicaid and federal/state/local regulations
Ability to effectively engage physicians, management level staff, colleagues, and stakeholders in order to build partnerships, achieve strategic initiatives, and attain organizational goals
Knowledge and expertise in utilization management and discharge planning
Excellent leadership skills. Visible leader with proven ability to coach, mentor and influence
Strong prioritization, problem-solving and critical thinking skills
Strong communication skills with diplomacy to establish rapport with all stakeholders
