What are the responsibilities and job description for the Behavioral Health Case Manager position at Bacon Street Youth and Family Services?
Bacon Street Youth and Family Services (BSYFS) seeks an enthusiastic professional to join our team! With direction from the Clinical Director, our Behavioral Health Case Manager will be instrumental in the implementation of program activities, therapeutic case management, and interventions for the Home First Williamsburg and Footprints Collaborative projects. The individual must have experience working with youth and families, specifically related to substance use and/or mental health disorders. Additional experience working with the homeless and/or precariously housed is highly beneficial. Experience with in home services highly preferred. Primary Responsibilities:
- Work in collaboration with an interagency team to help coordinate and implement the Home First Williamsburg and Footprints Collaborative projects, on behalf of Bacon Street Your and Family Services.
- Provide services for clients in need of mental/behavioral health services and are also either currently or have previously faced housing insecurity.
- Provides on-site/in home case management/therapeutic services for the Home First Williamsburg and Footprints Collaborative projects.
- Conducts intake interviews with new program clients, obtains data and develops treatment plans in collaboration with the clinical team members assigned to each client.
- Participate in mandatory training on trauma informed practices and be willing to advocate on behalf of clients in a trauma informed manner.
- Consults with representatives of other agencies or programs and provides and coordinates client case management services.
- Performs related duties and responsibilities as required.
Qualifications/Skills & Knowledge Requirements
Education: Bachelor’s Degree in a human services field
Experience: At least 2 years’ experience directly related to the duties and responsibilities specified. QMHP or Peer Recovery Support Specialist licensure preferred.
Environment/Physical Demands
Ability to climb stairs and be able to lift at least 25-50 lbs. Should have a driver’s license, their own transportation (mileage will be reimbursed for work purposes) and be comfortable meeting with clients in an in-home setting.
