The home visitor in the home-based program option is responsible for education, case management, and family advocacy to families in an assigned caseload by providing and linking families to comprehensive services including home visits and socialization activities. This position works with typically developing children and children with disabilities. Because this position is one part of a large system of services provided to children and families, this position also serves as a social service referral person and shares responsibility for parent involvement in the program.
- Assist pregnant women to access comprehensive prenatal and postpartum care,
through referrals. (EHS) - Follows a comprehensive Home-Based Curriculum Guide
- Works with parent to develop an individual plan for the child to include goal setting based on identified needs and prescriptions for objectives and activities to meet established goals (outcomes).
- Promote parental involvement in curriculum planning Policy Council, parent committees and other community groups.
- Screens, observes, and documents child observations in GOLD.
- Ensure an appropriate written Transition Plan for all children, six months before their third birthday; provide parents with training and information and establish communication to facilitate and coordinate the transition.
- Provide knowledge and expertise to Manager and team members (Home Visitors) relative to Home Based Program.
- Any other duties as may be required.
