What are the responsibilities and job description for the Manager, Case - Outreach/Veterans position at BakerRipley Career Page?
Reporting to the Case Management Supervisor, the Case Manager participates in outreach activities to ensure targeted clients are found and provided case management services in a timely manner. Utilizing a Housing First Model, the case manager assesses the needs of veteran households and with the client’s input, develops and implements a Housing Stability Plan leading to household stability and self-sufficiency. The case manager identifies community resources to promote the overall well-being of clients in care; determines veteran’s emergency needs and makes the appropriate connections to safe, affordable, and permanent housing.
1. The Case Manager (CM) completes full assessments with applicants to enroll SSVF-eligible veterans in case management services for a minimum of 90 days documenting all case management activities and events.
2. CM works to ensure veteran’s rapid entrance into permanent safe housing or towards keeping veteran stably housed and assists by obtaining the necessary support to prevent imminent homelessness or avoid going back to the shelters or unsafe living environments
3. CM mediates, advocates and leverages support for eligible veterans through intensive and effective case management, referring veteran and their immediate families to health care navigation, legal, employment, financial, and any other services identified by veterans and the entire family in care.
4. Timely and accurately documents and maintains records of all resources provided to clients, including providing all temporary financial assistance.
5. Manages client files safely and effectively and follows safety protocols and procedures to preserve the Personal Identifiable Information of all clients in care.
6. Exhibits high professionalism while advocating for the veteran and representing BakerRipley and the Veteran Services team during internal and external meetings and program-related events.
7. Performs other duties as may be assigned.
Bachelors degree preferred.
