Position Purpose and Summary

Under the direction of the department or project / program manager the Community Healthcare Worker (CHW) provides culturally appropriate general health education including but not limited to health, disease, and disabilities in American Indian Alaska Native (AIAN) communities. The position is responsible for client-focused outreach and home visit activities that supports medical and behavioral health services, as assigned. The CHW coordinates support to clients by linking them to medical, behavioral health, and community-based resources as well as provides case management and data tracking.

The CHW position encompasses a wide scope of duties including data entry, connect client with various services (housing, nutrition assistance, childcare, etc.), assist client with applying for health insurance, scheduling appointments, and assists with transporting duties, as needed.

Essential Duties, Functions & Responsibilities

  • Answer phones and text message patients in a professional manner.
  • Gather pertinent information regarding insurance coverage.
  • Clearly and concisely communicate with the patient and parent regarding post care steps as needed.
  • Properly document all patient charts and upload all records.
  • Facilitate the development of wellness focused skills among assigned clients as a member of the treatment team.
  • Serve as an advocate providing clients information and support.
  • Promote knowledge of available service options and choices.
  • Assist with teaching independent living skills such as accessing public transportation options, cooking, cleaning, accessing and maintain stable housing, personal finances, etc.
  • Recommend and make referrals to programs for instruction of life skills, livelihood skills, and workplace readiness skills necessary for successful reintegration into family life, the workforce, and the community as assigned by providers.
  • Case manages clients in group settings, individually or in their home as appropriate.
  • Monitor and keep case records on clients and report progress to supervisors; participate in wellness checks as assigned.
  • Provide clients with information about community resources and keep current on referral resources.
  • Attend required staff meetings and trainings as assigned.
  • Maintain client charts in a responsible manner and meet all paperwork deadlines as required by funding and accrediting agencies and agency policy.
  • Properly and safely transports patients and secures patient. Never transports patients in severe pain.
  • In accordance with the transportation of patients, use wheelchairs and other approved assistive devices to perform the essential duties of the position.
  • Demonstrates competency in transporting all specific age groups.
  • Ensures the safe timely and accurate transport of patients to their destination using the proper requested equipment.
  • Communicates effectively with nursing personnel, clinicians, technicians, or reception.
  • Ensures the comfort of the patient by being attentive. Checks modesty and only engages in appropriate conversation involving patients.
  • Alerts nursing staff immediately to any signs of patient distress or equipment malfunction.
  • Maintains equipment and reports equipment needing repairs.
  • Complies with regulatory agency standards, including federal, state and Accreditation Association of Ambulatory Health Care.
  • Adheres to facility confidentiality and patient’s rights policy as outlined in the facilities Health Insurance Portability and Accountability Act (HIPAA) policies and procedures.
  • Promotes vaccine awareness and administers as approved.
  • Performs community outreach via face-to-face, phone calls, and virtual platforms for education and assistance.
  • Supports the work of the organization by covering for Bakersfield location personnel as needed.
  • Performs other work-related duties as assigned by supervisor and / or management. Duties can be given either verbally or in writing.

  • Qualifications :

  • Valid California driver’s license and DMV report with fewer than two (2) infraction points within a 12-month period and automobile insurance.
  • Zero Major Moving Violations in the past 4 years.
  • Possession of a valid High School diploma / GED.
  • Certified Medical Assistant certificate preferred.
  • First aid / CPR certification at the BCLS level is required or obtained within three (3) months of hire.
  • Experience :

  • 1 year working in a medical office setting.
  • 1 year experience in a medical transporting preferred.
  • Demonstrates the ability to type 45 words per minute.
  • Knowledge of Electronic Health Record Systems preferred.
  • Case Management experience preferred.
