What are the responsibilities and job description for the LVN Emergency Room Case Management position at BAKERSFIELD FAMILY MEDICAL GROUP, INC.?
Under the direction of the Case Management Supervisor, this position is responsible for performing a wide variety of duties including evaluating patients' medical records and helping to facilitate, coordinate, and implement care for those patients who are discharged from the Emergency Room under Bakersfield Family Medical Group or Coastal Communities Physician Network. The Outpatient Emergency Room Case Manager will interact with other departments, clinic personnel, and outside providers in a professional and friendly manner, to create and maintain a positive relationship with our internal and external customers.
1.1 Receive Emergency Room patient list via email and/or fax from participating hospitals for BFMC or CCPN patients.
1.2 Work with Case Management Supervisor to determine daily patient assignments.
1.3 Obtain patient ER records from hospital EHR or request records from hospital Medical Records Dept.
1.4 Review patient ER records to determine post discharge care coordination needed including specialty and DME authorizations, follow up appointment with PCP, patient education, referral to Priority Care Clinic, Social Services, or high-risk outpatient case management.
1.5 Gather ER records from hospital EHR.
1.6 Organize ER records into their respective folders onto the G Drive in order for Health Information Management Dept to Upload into the patient's NextGen chart.
1.7 Fax patient ER records to assigned PCP office and obtain appointment if not already scheduled.
1.8 Contact patients to follow up on post discharge health status, provide patient education, advise of urgent care availability and nurse triage line and assist with post discharge care coordination needed.
1.9 Document all attempted outreaches in NextGen ER Care Management Follow-Up template.
1.10 Documentation should include hospital name, date and time of ER visit, reason for visit, action in ER, orders, case Manager contact, and interventions/care coordination performed.
1.11 Evaluate patient's support systems.
1.12 Ensure a smooth transition of care after discharge.
1.13 Participate in weekly IDT meetings telephonically to provide report on patients the week prior who meet criteria.
1.14 Serve as a clinical liaison for patients bringing necessary issues to the Chief Medical Officer and Case Management Supervisor.
1.15 Maintain a positive and team-oriented attitude for the benefit of staff morale. Promote and encourage teamwork and pride among staff members regarding Member Service functions and cultivate good interdepartmental relationships.
1.16 Attend staff meetings.
1.17 Cultural and Linguistics training required annually.
1.18 Proficient with Microsoft Word, Excel and Outlook Email.
1.19 Proficient with typing.
1.20 Other duties as assigned.
Requirements:
8.1 Graduate from an accredited Vocational Nursing program.
8.2 Current California LVN license.
8.3 Current BLS certification.
8.4 Minimum of two years clinical nursing experience.
8.5 Ability to utilize electronic mail effectively.
The pay range for this position at commencement of employment is expected to be between $30.15 and $35.47 however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, skills, and experience.
If hired, employee will be in an “at-will position” and the Company reserves the right to modify base salary (as well as any other discretionary payment or compensation program) at any time, including for reasons related to individual performance, Company or individual department/team performance, and market factors.
