Job Summary:
Bakersfield Post Acute is seeking a compassionate and detail-oriented Wound Care Nurse LVN to join our dedicated team. In this role, you will be responsible for assessing, planning, implementing, and evaluating wound care treatments for our residents. Your expertise will ensure that residents receive high-quality care that promotes healing and improves their overall quality of life. This position requires close collaboration with an interdisciplinary team to deliver comprehensive wound management in a skilled nursing facility setting.
Key Responsibilities:

Wound Assessment & Treatment:
- Perform thorough assessments of resident wounds, including size, depth, and type, to determine appropriate treatment plans.
- Develop, implement, and update individualized wound care plans in collaboration with RNs, physicians, and other healthcare professionals.
- Perform wound cleaning, debridement, and dressing changes in accordance with established protocols and best practices.

Patient Education & Support:
- Educate residents and their families on wound care management, proper dressing techniques, and strategies to prevent further complications.
- Provide ongoing support and monitor patient progress, adjusting care plans as necessary to ensure optimal outcomes.

Documentation & Compliance:
- Accurately document all wound care assessments, interventions, and patient responses in compliance with facility policies and regulatory guidelines.
- Ensure all practices adhere to infection control standards and regulatory requirements.

Collaboration & Communication:
- Work closely with the interdisciplinary team to coordinate comprehensive care for residents.
- Communicate effectively with team members to share updates, concerns, and treatment progress.

Quality Improvement:
- Participate in quality improvement initiatives related to wound care management.
- Stay updated on current wound care practices, techniques, and advancements, and integrate these into daily practice.
Qualifications:
Licensure & Certification:
- Must hold a valid California LVN (Licensed Vocational Nurse) license.
- Current Basic Life Support (BLS) certification is required.
- Additional wound care certification or specialized training in wound management is highly desirable.
Experience:
- Prior experience in a Skilled Nursing Facility or similar healthcare setting is preferred.
- Demonstrated experience in wound care management, including assessment, treatment, and documentation.
Skills & Abilities:
- Strong clinical assessment and wound care skills.
- Excellent communication and interpersonal skills, with the ability to work effectively within a team.
- Detail-oriented with strong organizational skills and the ability to manage multiple tasks simultaneously.
- A proactive approach to problem-solving and continuous improvement in care practices.
Working Conditions:
- This role involves direct patient care in a Skilled Nursing Facility setting, which may require adherence to infection control protocols and the use of personal protective equipment (PPE).
- The position may involve a standard shift schedule with occasional flexibility to meet resident care needs.
