Balance Point Wellness, LLC is accredited by the Accreditation Commission for Healthcare (ACHC). We are a community-based holistic mental health and wellness center that provides counseling/psychotherapy, psychiatry, adventure therapy, and wellness services to individuals and families in Maryland. We are located in Baltimore, Bel Air, Salisbury, and Hunt Valley, MD. We also have Telehealth division in Maryland, Delaware, and Pennsylvania, and a school-based program in Baltimore County and Harford County Schools.
A Mentor is expected to work with clients in office and in the community, provide behavioral support services, and crisis intervention for assigned clients, according to Individualized Treatment Plans.
Responsibilities:
- Provide mentoring and services to assigned clients as outlined in their plan of care.
- Meet with each minor client according to regulatory standards (at least three times a month in the home or community) to facilitate the development or restoration of life skills, including self-care, social/peer/family/teacher interactions, and independent living.
- Maintain accurate, detailed and current case records on each client as well as prepare reports and document case sessions per BPW policy.
- Complete administrative tasks as required.
- Attend regular staff meetings and supervision.
- Must collaborate with the client's therapist, prescriber, and other persons involved in the client's care.
- Be available to clients in the event of a mental health crisis while on duty and after hours.
- Carry out any other work-related functions requested by the supervisor.
- Responsible for following regulations of COMAR, ACHC, any other regulatory body, and BPW policies/ procedures related to service delivery and documentation.
Requirements:
- High School Diploma/GED; AA Degree or greater preferred in a health-related field
- Must be 21-years or older
- Previous experience working with children or adolescents in a social service setting.
- Valid driver's license with properly registered and insured vehicle with a clean driving record
- Must have own vehicle
- Proficient in Microsoft Office
- Ability to use good professional judgment. Detail oriented.
- Ability to work independently, with minimal direct supervision.
- Excellent organizational, prioritization, and interpersonal communication skills.
- Ability to handle multiple tasks and meet critical deadlines.
- Ability to meet short and long-term goals and project deadlines.
- Respect for the confidentiality of all communications required within job duties.
- Must pass a criminal background check and have no criminal convictions or history that indicate behavior which pose a potential threat to clients and staff
