A Non-Merit vacancy exists in the Department of Aging.

Non-Merit positions are not classified within the Baltimore County Government Classification and Compensation Plan.

Job Purpose 
The Baltimore County Department of Aging strengthens lives by connecting individuals to community resources, programs and services.

The Caregiver Support Program in Department of Aging has a Long-Term Care and Dementia Care Navigation grant to enhance services for persons with dementia.

As part of this grant, the Dementia Care Navigator will provide dementia screening, educational outreach programming, Options Counseling, and short-term case management to individuals with dementia and their families. 

Using a dementia journey navigation tool, the goal of the Dementia Care Navigator will be to provide individualized supports and connect individuals with dementia to appropriate resources.

Essential Job Duties

  • Works in conjunction with various BCDA programs to identify individuals with dementia who will benefit from the program. 
  • Uses a standardized tool to conduct dementia screenings for identified individuals who have not already received a diagnosis. 
  • Completes interviews by phone, in the home or another locale as needed to meet with clients and/or their support systems to assess for services and programs.
  • Using a dementia navigation tool, provides Options Counseling as needed to assist clients with long term support planning.
  • Provides follow up with clients and family to help ensure access to services and programs. Transitions clients to long term care case managers, if clients are moving to other programs and services.
  • Maintains individual case records, completes documentation and collects identified data.
  • Becomes a Certified Dementia Practitioner, a Dementia Live Coach, and a Dementia Friends Champion and conducts a variety of dementia education programs.
  • Assists with the collection and reporting of grant data and outcome measurements.
  • Provides backup assistance for the educational programs provided by the Caregiver Support Program and Dementia Friendly Baltimore County.

Required Education and Experience
Graduation from an accredited college or university with a bachelor's degree in a social or behavioral science.

Experience as described in the Essential Job Duties may be substituted on a year-for-year basis up to a maximum of four years for the required education.

Preferred Education 
Bachelor of Social Work (BSW)
Master of Social Work (MSW) 

Knowledge, Skills, and Abilities (Entry Level)
  • Knowledge of the theories and principles of human behavior and aging; including those related to dementia.
  • Knowledge of resources for older adults.
  • Computer skills and experience in word processing and email, and the capability to learn the functions of client case management software systems.
  • Writing skill necessary to complete forms, client documentation, and reports.
  • Interviewing, assessment, active listening, negotiation, problem solving, and public speaking skills.
  • Ability to gather and assess information and to apply guidelines for eligibility or suitability.
  • Ability to interpret program guidelines, policies, procedures, and regulations.
  • Ability to identify needs of individuals including crisis intervention.
  • Ability to make appropriate referrals.
  • Ability to communicate effectively orally and in writing.
  • Ability to establish and maintain effective working relationships with others.
  • Ability to conduct presentations to groups.
  • Ability to keep organized and remember details.

Knowledge, Skills, and Abilities (Full Performance)
  • Knowledge of the organization and role of assigned program within the County and State social and human services structure. 
  • Knowledge and understanding of applicable Federal, State, and local regulations, procedures, and guidelines. 
  • Knowledge of alternative resources for clients. 
  • Knowledge of County and departmental rules, regulations, policies, and procedures relating to performance of duties. 
  • Skill in gathering and assessing information and applying guidelines for eligibility or suitability.
  • Skill in interpreting program guidelines, policies, procedures, and regulations. 
  • Skill in identifying needs of individuals including crisis intervention. 
  • Skill in making appropriate referrals. 
  • Ability to develop service plans.

Licenses and Certificates
Possession of a valid driver's license equivalent to a noncommercial, class C Maryland driver's license is required at the time of appointment.

Applicants are required to submit proof of professional licenses, certifications and education beyond high school to meet the required and preferred qualifications of the position. Diplomas or transcripts must show the applicant's major field of study. Copies and unofficial transcripts are acceptable.

EXAMINATION PROCEDURE 
Applicants will be qualified based on an evaluation of their training and experience, as stated on their application, which includes answers to the supplemental questions. Applicants must state the dates and duties of past and present experience clearly and completely for evaluation purposes.This position requires satisfactory completion of County sponsored CPR and First Aid
certification trainings.

Applicants selected for this position must complete required position trainings and mandatory County and BCDA job related trainings.

Applicants selected for this position must maintain certifications throughout employment.

Physical and Environmental Conditions
This position is essentially sedentary with occasional walking, lifting, carrying materials or other restricted physical activities.

The position requires traveling between work sites.

Medical Examination and Employment Background Investigation
Applicants selected for employment with Baltimore County must successfully complete a physical examination, drug screening and employment investigation, including, but not limited to criminal background, education certification and fingerprinting checks.
