Job Description:
Provides person-focused therapeutic services for individuals experiencing mental and behavioral health events of varying acuity, inclusive of substance use and/or withdrawal. Contributes to the collaborative plan of care and delivery of services to individuals through assessment, evaluation, crisis interventions, referrals, education, linkage to care, and progress toward treatment plan outcome-based goals. Works as part of an interdisciplinary team with the individual as decision-maker.
Essential Functions:
- Work collaboratively with colleagues of the mobile services as well as community partners, providing support to individuals through the crisis and substance use recovery process.
- Work with individuals in crisis to develop a crisis plan, provide linkages to care, and introduce other crisis planning tools and professionals as appropriate and as needed, prior to the conclusion of the first engagement.
- Work with all members of the team to facilitate engagement of the individual who is in crisis; helping them feel welcome and comfortable.
- Work with individuals in crisis to assist with continuity of care by supporting connection to behavioral health, addiction, peer recovery, and other community resources.
- Builds relationships with addiction, behavioral health, peer recovery, community outreach providers supporting vulnerable populations, and other community resources.
- Performs timely and accurate documentation of all individual interactions for support, education, and advocacy, including clinically related information.
- Empower individuals to become self-advocates for their own recovery and wellness.
- Works with all individuals using a strengths-based perspective, providing support and encouragement.
- Coordinate and act as a liaison as assigned and appropriate for internal and external stakeholders.
- Demonstrate adherence to company policies and expectations and behavioral competencies, both explicit and implicit.
- All tasks, projects, and/or duties as assigned and authorized, as determined by your direct supervisor (or designated delegate) to be congruent with the role.
Qualifications
Education:
- High School Diploma, GED or Equivalent, Required
- Associate Degree in Human Services or related field, Preferred
Experience:
- Behavioral health and/or addiction lived experience, Required
- Knowledge of county behavioral health supports, Strongly Preferred
Additional:
- Valid and unrestricted driver’s license, Required
- Proficiency with Microsoft Office Suite Products, Required
- Proficiency and familiarity with EHR/EMR systems, Preferred
Credentials:
- Current BLS Certification, Preferred
