About The Role
The primary goal of this position is to support and empower Targeted Case Management/ Care Coordination (TCM/CC)_program participants and families by partnering with them to obtain needed community resources that will support their recovery, health, functionality, and well-being. The goal of TCM/CC Case Manager is to work collaboratively with participants returning from psychiatric hospitalization, problems with law enforcement, involvement with Department of Human Services and other psychosocial problems so that individuals and families can be effectively integrated into the community with an enhanced quality of life. This position will report directly to the Program Manager. In order to support these goals, the following objectives/responsibilities must be achieved:
Education
This position requires a bachelor’s degree in social work, psychology or a related human service field from an accredited college or university.
Experience
Experience working with individuals with mental illness, addiction and/or co-occurring disorders is given priority.
Additional
Applicant must have a valid MD driver’s license and will transport clients using a company vehicle when needed.

This position will report directly to the Program Manager. In order to support these goals, the following objectives/responsibilities must be achieved:
- Provide services in the least restrictive environment, case managers will be required to “meet participants where they are” meeting with them in their home or other identified community setting
- Maintain a case load of TCM/CC participant clients and provide specialized intensive case management services as needed
- Provide linkages to care and other community resources and supports to assist participants in maintaining recovery and preventing entrance into the criminal justice system
- Maintain a schedule with the ability to be flexible with hours when necessary, depending on the needs of participants being served
- Maintain collaborative relationships with community partners and support service agencies.
- Provide transportation to participants for appointments when necessary
- Complete timely and accurate documentation
- Complete outcome tracking forms for caseload of participants
- Complete Insurance/ Medicaid Authorizations for caseload
- Attend weekly, Care Management meetings to discuss the progress of TCM/CC participants
- Attend trainings and conferences as needed to improve program skills
- Additional responsibilities as determined by the Program Manager
