An Acute Services Counselor Practitioner works under the direction and supervision of a Clinical Director, providing behavioral health services to individuals with mental illness, substance use issues and/or co-occurring disorders as part of crisis intervention , stabilization and support. The position includes the provision therapeutics activities such as brief intervention and core functions of care management as advocacy, referral and coordination of services as part of the continuity of care upon discharge. This position requires a high degree of self-initiative and motivation. The schedule is flexible, as determined by client needs, and may involve working on weekends. In addition, the Acute Services Counselor receives proper training and participates in all utilization review protocols related to the authorization process from health insurances.

ESSENTIAL FUNCTIONS

1. Assesses persons served progress in working toward the resolution of problems and in working toward service and treatment goals.

2. Maintain an on-going relationships with community agencies and function as a liaison with identified individuals.

3. Assesses persons served needs in all service areas together with the person served; develops a service plan of care , within the required time frames; documents persons served participation with appropriate notations in the person served record; obtains all needed signatures in a timely manner.

4. Assesses the persons served home environment when needed and level of functioning within the family by interviewing the family

5. Links the persons served with needed services, such as educational, legal, medical, social, and vocational and any other services identified as being needed in the service plan.

6. Advocates for the person served with all service providers, bureaucracies, governmental entities, and in general in order to achieve successful outcomes in obtaining services and other goals.

7. Provide instruction and lectures on therapeutic topics; provide psycho-educational modules and facilitate therapeutic process groups and/or brief interventions tailored to the person served needs based on approved Evidenced Based Practices

8. Reports regarding behavioral observations and provide appropriate clinical interventions for managing client’s conduct;

9. Administers all outcome evaluation instruments in a timely manner.

10. Maintains record integrity by documenting all services provided to the person served in accordance with Center Medical Records practices and all applicable laws.

11. Reports services provided in the appropriate billing generating documents in a timely and strictly, ethical manner. including timely and accurate completion of client notes and status within their files

12. Actively participates in supervisory conferences and in agency committees and community activities as assigned by supervisor.

13. Reports all unusual incidents according to the agency policy.

14. Make sure that corresponding documentation is being completed for voluntary and involuntary patients.

15. Organize records following the CSU Form-File Audit ( unless is under EHR).

16. Collect any missing signature such as doctor’s signature, nurses, case workers, supervisors and others.

17. Keep CSU records after patient’s discharge and send them to clinical record after being completed, organize and audit.

18. Check the census book; oversee if all the information is correct.

19. Check clients insurance and assist in getting authorization from insurance careers.

20. Conduct assessments and coordinate ongoing care related to funding, level of care assessments for Medicaid individuals, review and make recommendations and assist with ongoing UM/monitoring of care as it pertains to medical necessity for all cases.

21. Works closely with the courts (Marchman Act/ Jail Diversion) and other forensic programs in the community.

22. Completes necessary documentation so that the person served can be placed on waiting list for all residential treatment programs in the community.

23. Meet/update all training/certification requirements of position.

24. Assume other responsibilities related to CSU as requested by the Supervisor or Program Director.

25. All other duties as assigned.

EDUCATION AND/OR EXPERIENCE Bachelor’s degree from an accredited college or university preferably major course work in Counseling, Psychology, Social Work, Criminal Justice or related discipline in the human services field. A minimum of two (2) years of experience working with adults with serious emotional disturbances, preferably in Crisis Behavioral Services. A Master level can commensurate for one year of experience. Bilingual (English/Spanish) is a bone fide qualification.

LANGUAGE SKILLS: Ability to read, write and speak in English and Spanish (or other languages) helpful. Must be able to read and comprehend simple instructions, short correspondence, and memorandums. Agility to effectively communicate information both orally and written on one-to-one and small group situations to clients and other employees.

MATHEMATICAL SKILLS: Ability to add, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio and percent.

REASONING ABILITY: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form.

ORGANIZATIONAL SKILLS: Must be organized and able to schedule work load appropriately without hindrance to client appointments etc.

DECISION MAKING: Must be able to work independently with competent decision making using established criteria and guidelines.

OTHER SKILLS AND ABILITIES: Computer Skills: To perform this job successfully, an individual should have knowledge of Accounting software; Database software; Internet software and Word Processing software.
