Summary
Provide high quality assessment, intervention, and consultation within an integrated primary care team. This behavioral health consultant will provide brief, targeted diagnostic assessment and interventions within an interdisciplinary team. Focus on providing rapid access to high quality and evidence-based interventions to address a broad range of mental health concerns and substance use in addition to treating behavioral aspects of chronic medical illness. Ideal candidates enjoy a fast-paced work environment and are passionate about working in a community health setting serving adults and children with comprehensive person-centered care. Ideal candidates have advanced training working in an integrated primary care setting, are comfortable providing treatment across the lifespan, and enjoy providing both in-person and telehealth services to increase access to care.

Provides brief evidence-based interventions for mental health, substance abuse, chronic medical conditions,
and population health.
Warm handoffs
Curbside Consults
Integration of identified patients in primary care to outpatient behavioral health
Crisis intervention and de-escalation
Brief evidence-based consults approx. 15 minutes in duration with prolonged evaluations and interventions as clinically needed and documented
Biopsychosocial assessmnents
Evidence based care
Measurement based care
Actively engages in clinical supervision with designated supervisor and/or mentor
Actively participate in meetings that support Banyan Health Systems’ integrated health care model to provide comprehensive care for clients
Performs evaluations by interviewing patient to obtain information concerning their problems and functioning and reviewing relevant screeners
Provides Psychoeducation to patients on wellness, prevention, treatment, illness, and medication adherence.
Collaborates with primary care providers and other team members to develop comprehensive and unified treatment plans to address patient needs.
Provides evidence informed interventions to assist patient and the interdisciplinary team in managing symptoms and improved functioning
Monitors symptoms and functioning, response to care plan, and need for revisions to care plan based on readiness to change, social and contextual factors, and level of engagement
Communicates and collaborates with relevant community entities serving patients and families
Engages patients in services in person and via telehealth
Completes documentation in EHR same day or within 24 hours per BHS’ Documentation Policy
Is available for crisis assessment and management
Follows BHS ‘Compliance policies and procedures, including reporting of abuse and/or neglect
Collaborates with other care providers concerning patients and families
Maintains highest quality patient care by providing a safe environment through ethical and professional conduct
Supports BHS’ organizational mission and initiatives, including quality improvement, assurance, and patient safety and risk management processes.
Psychiatric and medical providers will provide medication management for mental health and medication assisted treatment for substance use disorders as clinically appropriate.
Additional roles and responsibilities may be included at the need and discretion of the clinic and supervisors.
Qualifications
BHP Level 1 : Licensed Master's degree in Social Work, Marriage and Family Therapy, or Mental Health Counseling.
Certificates and Licenses
Must be licensed as a Florida Psychiatrist, Psychologist; Psychotherapist: Clinical Social Worker, Marriage and Family Therapist, or Mental Health Counselor; Doctor of Behavioral Health (licensed by designation)
Communication Skills
Ability to provide brief, target assessment of DSM5 TR diagnoses, tailor evidence-based treatments to the individual needs of the patients, collaborating with providers, and respond effectively and sensitively to a broad range of patient and family concerns. Ability to communicate and collaborate effectively with relevant outside entities regarding patient and family needs.
Computer Skills
To perform this job successfully, an individual should have knowledge of Word Processing software, Electronic Medical Records software, and Spreadsheet software. Ideal candidates would have experience with concurrent documentation during clinical care.
Supervisory Responsibilities
May supervise psychology, social work, and/or school therapy students. Supervision depends on clinician, location, and experience.
