Summary of Responsibilities
The MAT Care Coordinator assists individuals in navigating through the medical, behavioral health and social service systems by working with the client, multidisciplinary team, and other resources to facilitate the appropriate delivery of behavioral and physical healthcare. The care coordinator serves as a guide who is responsible for overseeing the community-level team process and takes the lead on cross-system/agency collaboration, pooled resources, client voice and choice, and consensus building. The care coordinator works with and guides the team process and tasks, while building collaboration with all parties at the table.
Essential Duties and Responsibilities
· Identify service needs, gaps and barriers to treatment to direct resources for the client, as needed.
· Coordinate referrals to providers to ensure the provision of behavioral health, medical, housing and social supports for clients.
· Remain in contact with the client and treating providers to ensure communication about individual progress and/or needs while assisting to eliminate barriers to treatment.
· Provide client with information about referrals to services.
· Follow up on client’s referrals to ensure successful linkage.
· Identify, develop and maintain relationships with treating providers, key specialist groups, hospitals, and community agencies to leverage resources, share data and address system barriers.
· Keep daily documentation of coordination of care efforts for each client, according to policy.
· Maintain data of outcomes.
· Applies evidenced bases practices, such as Motivational Interviewing, to increase client engagement and retention.
Bilingual is a must!!!!!!
Education, Experience, and Skills Required
· Bachelor’s Degree in human services or related field from an accredited university.
· A minimum of one year’s experience working within a mental health and/or substance abuse and/or primary care setting.
· Experience with Motivational Interviewing is preferred.
· Ability to speak in English and Spanish (Spanish is preferred). Must be able to read and write in English.
· Must have good organizational skills in the preparation of daily work schedules and the execution of duties and responsibilities.
· Must be able to work independently and manage multiple tasks.
· Must be able to effectively work in a collaborative manner in order to impact successful and effective treatment delivery.
· Must be proficient with computers (Electronic Health Records, spreadsheets, word documents, electronic mail).
