The Clinical Case Manager is responsible for performing a wide variety of patient care activities as directed by the Executive and Clinical Director. The Clinical Case Manager will collaborate with the Treatment Team to formulate the Master Treatment Plan, and assists in coordinating discharge planning. The Clinical Case Manager could have the responsibility for maintaining communication with the referral source and other services provided as needed. He/She will be responsible for providing support to members of the Clinical Team.

Job Duties
- The Case Manager will be the liaison between the client and the community.
- Will be a link for clients by finding them volunteer work and community involvement.
- Will help clients work on resumes and look for work via Craigslist and on the internet.
- Will organize events for the clients in and around the community.
- Will organize outings, projects and field trips for clients.
- Will have access to guest speakers for groups for clients (NA, AA).
- Will hold monthly trainings for clients on :
- Universal Precautions,
- Client rights
- Emergency procedures
- Grievance procedure
- Infection Control
- STD’s/HIV/AIDS
- Client wellness
- Will coordinate outside Dr. appointments for clients
- Will work closely with the MD in obtaining client medication prescriptions.
- Will keep in contact with the pharmacy to ensure timely delivery of client medications.
- Will work directly with the clinical Director for ideas, strategy and issues relating to clients.
- Will help clients with discharges, i.e. referring them to appropriate ancillary services working closely with the Clinical Director and specific therapist.
- Manage bus passes for clients.
- Manage client funds
- Complete disability, food stamp and unemployment applications
- Complete orientation step downs from PHP to IOP
- Track and manage client Rent receipt
- Will maintain a list of current client that need to see MD
- Complies with facility policies and procedures.
- Will participate in any and all trainings required for BTC staff according to regulatory guidelines.
- Participates in all staff meetings; responsible for obtaining information if not in attendance.
- Will work directly with the Clinical Director for ideas, strategy and issues relating to clients.
- Approaches clients/family members, significant others, and visitors with a respectful caring manner to convey a sense of concern and warmth.
- Maintains confidentiality of client information and records; and of facility proprietary, privileged, and/or confidential information.
- Communicates and problem solves via the chain of command.
- Delivers care in a non-judgmental and non-discriminatory manner, sensitive to client and staff diversity.
- At a Minimum Associates Degree or equivalent preferred
- 2-4 years of of work experience in substance abuse and mental health field preferred
- Experience in conflict resolution
- Compassionate and caring demeanor
- Ability to build rapport with clients
- Excellent written and verbal communication skills
- Familiar with community resources
- Experience with KIPU EMR preferred
- Knowledge of principles and practices of the discipline.
