Education
- BSW required, MSW, LMSW preferred
- Previous experience in adult long-term care preferred.
- Two or more years of experience working in a residential facility.
- Experience in individual/family counseling, group facilitation, admission and discharge planning and care coordination.
Skills

- Ability to provide case coordination, admission and discharge planning with an awareness of available community services.

Essential Functions of the Job:
- Participate as a member of the interdisciplinary care team responsible for assessment and care plan development.
- Facilitate the admission process of new patients, ensuring that they and their responsible parties are aware of their rights and responsibilities as patients of facility.
- Assist patient and family members in understanding and executing advance directives.
- Maintain contact with the patient and family regarding the patient’s problems and rights.
- Provide referrals as appropriate when there are financial, legal or other problems requiring the assistance of community experts.
- Assist patients in understanding their Social Security, Veterans and Medicare & Medicaid rights. This includes assisting them with necessary documentation to ensure that they receive appropriate payments and entitlements, including Medicare part D (prescription coverage).
- Assist patients in utilizing individual and group activities, finding options to meet their social and emotional needs.
- Observe, record and notify staff of emotional and psychosocial changes in mood, behavior and personality. Confer with staff, patients and family members regarding changes in the care plan to ensure that these changing needs are met.
- Utilize screening, counseling, crisis intervention, and referrals to respond to the needs of patients and family members, including issues of transition, aging, and grief and loss.
- Maintain up-to-date records of contacts, care plans, and other pertinent information concerning patient and family problems.
- Complete care plans, MDS, and other required documentation in a timely manner.
- Act as a resource for staff in understanding the emotional needs of patients and how to provide support, maintain individuality and ensure dignity.
- Assist patients and family members in exercising their rights to make complaints and file grievances.
- Facilitate the discharge process to ensure a safe transition.
- Remain current with advances in professional practice and any license or certification requirements.
- Participate in supervision meetings and training as required.
- Develop and co-facilitate activities and programs for patients and family members, in conjunction with other departments and professionals such as Rehabilitation, Activities, and the Chaplain.
- Other duties as assigned.

Physical and Mental Requirements:
*Ability to read and write at the level of Grade 12 or above.
*Must be able to hear alarms and intercom pages.
*Must be able to communicate orally and in writing.
Working Conditions
