Social Worker
Baptist Health Oncology/Hematology
Full Time 40 hours weekly dayshift
Job Summary: Provides social work services to patients and families as requested by physicians, staff, or patients and families themselves. Services include psychosocial assessment, individual or family counseling, assistance with assessing and acquiring financial and other needed resources, education, problem-solving and crisis intervention. Documents all contacts with/on behalf of patients and families. Provides consultation, training and support to staff as needed. Participates in in-service and other continuing education opportunities and regular supervisory sessions to review expectations and develop professionally. Liaison with social services at BH LAG, BH LOU, BH Eastpoint, and patient advocacy groups in the community.
Principal Duties and Responsibilities:
The following is a summary of the major functions of this individual's job. He or she may perform other duties, both major and minor, which are not mentioned below, and specific functions may change from time to time.
1. Psychosocial assessment of newly diagnosed cancer patients
2. Individual counseling and psychosocial support for cancer patients and/or family members
3. Crises intervention for patient and/or family as needed
4. Evaluate need for financial / medication assistance and facilitate access to appropriate resources
5. Facilitates management of patient assistance funds
6. Collaborates with community organizations related to hematology/oncology patient population.
7. Resource for CBC, radiation oncology department staff, and oncology program staff.
8. Engages in patient navigation for oncology patients by: Providing individualized assistance to patients, families, and caregivers to help overcome health system barriers and facilitate timely access to quality medical and psychosocial care through all phases of the cancer experience.
Minimum Education, Training and Experience Required:
- Must have Master’s degree in Social Work.
- Must be licensed as a Licensed Clinical Social Worker in the State of Kentucky
- Prefer experience with EHR.
