After-Hours Registered Nurse Job Description

We are seeking a compassionate and skilled After-Hours Registered Nurse to provide end-of-life care to patients and their families in the comfort of their own homes.

Responsibilities:

• Conduct comprehensive and timely delivery of hospice care for patients and their families after regular business hours, including at night and on weekends.
• Triage patient needs over the phone until a nurse can provide an in-person visit.
• Respond to crisis calls from patients and families with urgency and compassion.
• Make initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
• Perform after-hours visits, including admission visits, death visits, recertification visits, and routine visits.
• Identify and address patient/family physical, psychosocial, emotional, and environmental needs.
• Document problems, goals, interventions, and patient/family response to hospice care.

Requirements:

• One year of clinical experience as a Registered Nurse.
• Current RN licensure in the state of practice and one year of clinical experience.
• Current CPR Certification.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
