The Social Services Director is the social service designee of the facility. The social service designee will be responsible to plan, organize, establish, administer, and implement a Social Services program that will reflect the Purpose, Mission, Vision, Values, and Promise of Baptist Homes and Healthcare Ministries and cooperate with all disciplines to provide for the maximum physical, mental, emotional, social, and spiritual well-being of each resident.

RESPONSIBILITIES AND TASKS

· May include conducting pre-admission interview to gather pertinent information about applicants and provide information about Baptist Homes

· Upon admission, help the resident adjust to Baptist Homes, including familiarizing them with the facility, review of resident rights and Resident’s Handbook and the completion of necessary Social Service documentation.

· Prepare and maintain a written social history-individualized program to meet the needs of each resident to be used by Baptist Homes staff.

· Document an ongoing activity history for each resident.

· Plan, provide or arrange for medical transportation.

· Help residents to exercise their rights as citizens including arrangements for voting in elections when the Activities Director isn’t present.

· Serve as a liaison between residents and supportive services (i.e., hospitals, family services, veterans and other government and social services, etc.).

· Communicate with resident’s contact persons on an “as needed” basis.

· Assist the Director of Nursing with the Resident Assessment Instrument.

· Prepare a written semi-annual review for each resident regarding Social Service history.

· Work with Baptist Homes Case Management Ministry as needed.

· Participate in IDT meetings.

· Participate in and report at staff meetings.

· Assist in writing promotional articles as opportunities arise.

· Be available to represent Baptist Homes to organizations and individuals as opportunities arise.

· Assist in planning and conducting in-service training programs.

· Assist resident/family with the Medicaid process.

· Complete and submit DA-124 as requested.

· Contact ombudsman as needed.

· Complete bed hold policy.

· Willing to perform other duties in the best interest of BHTC as assigned by the administrator

· Workplace Safety: Practices safety, including, but not limited to, proper hand washing, obeying OSHA, State and BHHM workplace safety regulations, policies, and procedures, and maintaining an uncluttered, clean, and safe work environment.

QUALIFICATIONS

· High school diploma or GED.

· Preferred: BA or BS from an accredited college or university in social work or related field or LTC fieldwork experience.

· Familiar with the Department of Health and Senior Services regulations in Social Services.

· Exercise good listening skills.

· Ability to relate to residents on a feeling level and to deal with emotional issues in a non-judgmental and consistent manner.

· General understanding of the aging process and the changes associated with aging.

· Knowledge of available community resources.

· Ability to exercise good judgment in problem solving and to remain objective.

· Obtain and maintain a current MO Chauffeur’s License. Excellent vehicle driving record and the ability to transport residents.

· Ability to advocate for all residents, impartially, and keep resident information confidential.

· Sufficient typing, computer, and documenting skills.

  • 8 hour shift
