About the Role:
- Conduct psychosocial assessments of patients and families to determine support and discharge planning needs.
- Develop and coordinate discharge plans with other healthcare providers to ensure a smooth transition from one level of care to another.
- Provide supportive counseling to individuals, families, and groups to address identified concerns and enhance understanding of coping with illness/hospitalization.
- Maintain current knowledge of community resources, placement facilities, and financial agencies to meet patient and family needs.
Requirements:
- Master's degree in social work (MSW) or related field.
- Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) preferred.
- At least 1 year of experience in a medical or clinical setting.
- Excellent communication, conflict management, organizational, and planning skills.
