About the Job:
We are seeking a skilled Patient Advocate Specialist to join our team at Baptist Memorial. As a key member of our healthcare team, you will provide critical support to patients and families, ensuring their emotional, psychological, and practical needs are met.
Duties and Responsibilities:
- Conduct thorough psychosocial assessments of patients and families to determine support and discharge planning needs
- Coordinate and collaborate discharge plans for patients and families with other healthcare providers
- Provide supportive counseling to individuals, families, and groups to address identified concerns and enhance understanding of, coping with, and adjustment to illness/hospitalization
Requirements:
- Master's degree in Social Work (MSW) - minimum requirement: Licensed Social Worker (LSW)
- Preferred experience: 3 years in clinical social work
