SummaryManages the care of a specific group of adult patients providing behavioral health care within BMG / provider-based hospital clinics. Provides a wide array of social work services, including psychosocial assessments and screenings. Contacts patients to coordinate community resources for identified social determinants of health risk factors. Collaborates with the patient's care team physicians, nurses, and allied health professionals regarding behavioral health and social needs.Responsibilities-Utilize social work skills by conducting psychosocial assessments and mental health evaluations.-Provides supportive and educational counseling and interventions through visits and tele-medicine.-Provides counseling to individual patients, groups, or families.-Provides psychotherapy crisis services.-Consult with physicians on patients' psychosocial assessment.-Analyzes patients' social determinant of health needs to identify risks.-Plans educational activities.-Collaborates with the physician and all members of the multidisciplinary team to facilitate care and assist in meeting goals.-Maintains current knowledge of and accessibility to community resources, community programs, and financial agencies to meet patient and family needs.-Completes assigned goals.-Complies with policies that support patients' fundamental rights to considerate care, protect their personal dignity and respect their cultural, psychosocial and spiritual values in an effort to meet patients' care needs and preferences.Requirements, Preferences and ExperienceEducationPreferred : LCSWMinimum : Masters of Social Work LMSW- must be licensed per state requirementsLicensure, Registration, CertificationPreferred : CCM;ACM;LMSWMinimum : LMSWSpecial SkillsMinimum : Excellent interpersonal communication and negotiation skills.
