Job Summary:
We are looking for a Therapist-Speech I PRN who can evaluate and assess patients to establish functional, measurable goals. You will provide treatment according to a written plan of care to meet established goals and communicate findings and progress accordingly.
Duties and Responsibilities:
- Evaluate and assess patients based on subjective and objective information
- Communicate results of evaluation to patient and other caregivers
- Provide treatment according to established goals on plan of care
- Document interventions/care in the patient's medical record
- Evaluate effectiveness of plan of care and reassess patient's functional status as needed
- Provide discharge planning information which includes patient/family education, follow-up, and referral to community resources
- Contribute to the development of staff and students
- Participate in activities designed to improve health care delivery
Requirements:
To be successful in this role, you must have a minimum of Entry level experience and hold BLS certification within 14 days of hire date. Additionally, you should possess basic computer literacy skills such as keyboarding, using a mouse, and navigating sites.
