Job Summary

Key Responsibilities
- Conduct initial and comprehensive nursing evaluations to determine primary focus of care and develop individualized plans
- Coordinate total plan of care with other healthcare professionals and ensure continuity of patient care
- Provide high-quality nursing care in accordance with physician orders and agency policies
- Ensure patients meet home health eligibility and medical necessity guidelines
Requirements
- Current RN licensure in the state of practice
- Ability to build trusted nurse-patient relationships
