We believe mental & emotional health is an important part of overall health and wellness. Our behavioral health providers work closely with patients and their Primary Care Providers to help them meet their health care goals.
We are seeking a Case Manager for our expanded behavioral health program in Barbour County at both Belington Clinic and Myers Clinic sites. Through this position, you will work directly with patients and their families in our Integrated BH program to coordinate care and referrals to wraparound services that support clinical care. The Case Managers make proactive contact to improve patient outcomes, increase patient compliance, improve patient recall and retention, enhance patient advocacy, and develop cooperative relationships with other health and social services agencies.
Job Duties & Responsibilities:
- Provide comprehensive care coordination and support services for clients.
- Schedule mental health and medical appointments in accordance with established treatment plans.
- Conduct intake assessments, including psychosocial evaluations and related psychiatric templates.
- Obtain necessary preauthorizations from insurance providers for billing purposes.
- Ensure that all behavioral health patient records, including consents and assessments, are current and accurately maintained within the EHR system.
- Process requests for Behavioral Health medical records in a timely manner.
- Maintain thorough and confidential documentation of patient interactions and progress.
- Coordinate Treatment Team meetings, as required.
- Participate in regular staff and departmental meetings.
- Perform other duties as assigned by the Chief Behavioral Health Officer (CBHO), Lead Behavioral Health Case Manager, or Chief Executive Officer (CEO).
Minimum Job Requirements:
- The ideal candidate must demonstrate the ability to perform each essential duty satisfactorily.
- A minimum of a bachelor's degree in psychology, human services, social work, or a related field from an accredited institution is required.
- A minimum of two to three years of relevant experience is preferred, but not required.
- Familiarity with and an interest in community health and behavioral health practice environments.
- High levels of professional maturity, a commitment to excellence, reliability, and dependability, along with strong independence, flexibility, initiative, and time management skills are needed.
- Proficient computer skills are required; experience with electronic health record (EHR) programs and software is preferred.
Knowledge, Skills, & Abilities (KSAs):
- Strong patient assessment and crisis intervention skills.
- Familiarity with medical and pharmacology terminology.
- Excellent verbal communication skills, both in person and over the phone.
- Ability to establish and maintain effective relationships with clients to monitor their progress.
- Exceptional organizational and documentation skills.
- Capacity to work effectively with diverse and challenging populations.

Salary Range:
- Based on levels of experience and educational training
Job Status:
