Job Overview
Professional and technical work in planning and implementing regional programs, developing, and implementing strategies, and coordination of activities related to certain grant funded programs in accordance with County’s goals and State and federal requirements and guidelines; all other related work, as required.
Duties
- The YYA (Youth and Young Adult) Coordinated Entry (CE) Project Specialist assists Barnstable County Department of Human Services in coordinating regional YYA homelessness strategies ensuring that the CE system is “a centralized or coordinated process designed to coordinate program participant intake assessment and provision of referrals. [Such a] system covers the [CoC’s] geographic area, is easily accessed by individuals and families seeking housing or services, is well advertised, and includes a comprehensive and standardized assessment tool. (24 CFR part 578.3).”
- Collaborates with the community to expand partnerships across youth-serving systems and providers to ensure that YYA are connected to CE and supported to identify housing and services interventions of their choice.
- Facilitates case coordination efforts to identify YYA with high level needs and connect them to services and resources of their choosing.
- Must be able to develop or possesses expertise on the Coordinated Entry System and our Continuum of Care’s regulations, policies and procedures. Remains current on emerging trends, amended regulations and HUD guidance.
- Works collaboratively with the Homeless Management Information System (HMIS) and Continuum of Care (CoC) Program Managers to encourage accurate and timely data entry by partner agencies and actively monitor the database for errors.
- Trains partner agencies on how to enroll YYA in the YYA Coordinated Entry System.
- Provides partner agencies and any interested party with reporting on data collected through HMIS or other data sources.
- Builds relationships with local and state community resources, with a focus on maintaining knowledge of how to access these resources correctly for housing participants.
- Monitors peer-reviewed research, gray literature, and the media for information and evidence-based practices that are relevant to homelessness services.
- Creates, shares, and disseminates YYA homelessness materials, resources, and information to increase public awareness of services, including housing options and support services related to underlying causes of housing instability.
- Supports HMIS and CoC Program Managers in completion and submission of monthly and annual reports, strategic plans, logic models, and other required reporting instruments.
- In concert with the HMIS and CoC Program Managers, participates in local, regional, and state-level discussions regarding homelessness.
- Coordinates work with other Department staff and consultants on the issue of YYA Homelessness and related issues.
- Assists HMIS and CoC Program Managers with compiling, organizing, and writing of reports and makes presentations on data and research findings to non-profit organizations, local and state officials, the media, and the general public.
Qualifications
- Bachelor’s Degree from an accredited college or university in relevant field (e.g., public administration, public policy, social work, social sciences); minimum of two years’ experience working on housing/homelessness policy and programs; or an equivalent combination of education and experience.
- Knowledge of housing/homelessness systems and resources in the region, evidence-based practices, data collection tools and data driven planning, coordination of multi-sector stakeholder groups and group dynamics/processes.
- Ability to plan, organize and determine best method for delivery of services. Ability to work on multiple projects simultaneously and meet deadlines; responsible for assignment completion and follow through; comfortable working collaboratively as part of a group/team. Ability to disseminate complex information in a meaningful way, engage stakeholders, and others. Ability to succeed in a fast-paced and results-driven work environment.
- Skill: Well organized. Strong problem-solving ability, oral and written communication skills. Skill in establishing and maintaining effective working relationships with a variety of community partners to build capacity to address Youth and Young Adult Homelessness. Strong proficiency with Microsoft Office software, specifically Word, Excel, PowerPoint, and Teams. Ability to learn proprietary software.
