Provides case management, court services and protective services to individuals and families who are experiencing complex problems that affect physical and emotional well being. Target groups served includes people across their life span that need support for recovery from AODA and Mental Health issues. This position is primarily responsible for case management of Comprehensive Community Services. This position works under the general direction of the Program Manager.

EXAMPLE OF DUTIES
Provide case management to assure accomplishment of client/agency goals. Provide information and referral to maximize resources of clients. Electronically documents all service contacts. Complete comprehensive assessments of client needs through client interviews, collateral and resource contacts, using standardized tools. Identify possible resources. Provide rationale for resource preferences. Project time lines for goal achievements. Develop and review written case plans focusing on goal attainment and monitor compliance with regulations. Provide on-going service coordination to include electronic documentation of same. Attend regular service provider staffing meetings. Provide timely documentation of case closures. Consult with legal counsel on appropriate case specific matters and provides information for court petitions. Prepare court reports and related documents, attend hearings, provide testimony, and monitor compliance with orders, as appropriate. Utilize available data systems, providing information per agency basis. Check client data files for service strips to ensure accuracy of information. Authorize bills for payment per client plans. Complete daily billing records. Understand relationship between client files and allowable costs. Develop and maintain positive working relationships with clients, co-workers and community resources. Demonstrate ability to work independently and as a team member. Respond timely to all telephone contacts and mail per agency policy. Is knowledgeable about and follows State statutes, administrative code, standards and policies, County and Department of Health and Human Services policies and procedures and social work practice standards. Coordinate efforts with community agencies to improve working relationships and to provide better and more coordinated services to clients Participate in Intake functions, screens calls, provides information, provides immediate intervention services to individuals and families in crisis for new referrals, and investigates abuse/neglect allegations for adults and children. Provide access to services to include information and referral and problem solving with clients. Participate in monthly conferences with supervisor. Obtain case consultation from supervisor and participates in regular unit meetings. Attend other staff meetings as scheduled. Participate in staff development/training activities to enhance skills as well as maintaining up-to-date knowledge base of social work issues and techniques. Update manuals regularly per supervisory instruction. Perform other duties as assigned.

SUPPLEMENTAL INFORMATION
The individual must possess the skills and abilities to: Demonstrate consistent growth in performance, moving toward independent case management within the first 12 months of employment. Plan and organize work; set priorities and adhere to timelines. Relate to people in an unprejudiced and understanding manner with concern for individual circumstances, cultures, and preferences. Prepare and maintain necessary records and reports. Understand and follow oral and written instructions Work cooperatively and effectively with other service providers/organizations. Communicate effectively and appropriately with supervisor, co-workers, clients, etc. Assess, evaluate, and set goals with individual clients to enhance functioning in concert with the individual's preferences. Communicate with a wide variety of individuals with diverse backgrounds, abilities, and values. Respond to common inquiries or complaints from clients, regulatory agencies, or members of the provider community. Write documents that conform to prescribed style and format. Add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Define problems, collect data, establish facts, and draw valid conclusions and interpret an extensive variety of technical instructions. Work with automated data systems including word processing on personal computers. Develop knowledge of current social and economic problems and their impact on families and individuals; knowledge of community human service resources and means of access by people in need; and familiarity with literature in the field of social work. Utilize interviewing methods for obtaining complete and accurate information when working with families in financial and social crisis situations. Ability to maintain professional creditability for court appearances and working with caseload. Possess strong written and verbal communication skills.
