Hourly Range: $49.95 USD to $77.35 USD 

Summary of Position:
Provides professional rehabilitation patient care for home health and hospice clients in the privacy of their own home utilizing established policies and procedures, ensuring safe and effective care. Services are provided based on the ages of the population served; Young adult 18 years through geriatric – 65 years or older and in collaboration with the multidisciplinary care team.


Qualifications
Education:
● Graduation from an accredited Physical Therapy program with a baccalaureate, masters or doctorate degree in Physical Therapy and successful passage of the national licensure exam.
Experience:
● Minimum of Two (2) years’ experience as a Physical Therapist preferred.
● Prior Home Health or Hospice experience preferred.
Knowledge/Skills/Abilities:
● Proficient computer skills as are required to document, communicate and enter information into the electronic medical records system.
● Requires critical thinking skills, decisive judgment and ability to work with minimal supervision.
● Must be able to work in a stressful environment and take appropriate action.
● Excellent oral and written communication skills.
● Excellent interpersonal skills.
● Ability to collaborate with various departments in and outside of the Barton Health System in order to provide consistently exceptional patient care.
● Attention to detail, meticulous development and maintenance of regulatory paperwork.
● Demonstrates high level of personal and professional accountability and responsibility.
● Must have the proven ability to maintain confidentiality.
● Must demonstrate excellent customer service and communication skills.
● Must be flexible, dependable, and demonstrate the ability to adapt to change.
Certifications/Licensure:
● Current Physical license in California and Nevada or ability to obtain prior to hire date
● Current CPR/BLS certification by the American Heart Association (BLS for the Healthcare Provider) or ability to obtain prior to start date
● Valid California or Nevada Driver’s license (depending on state of employee’s residence) or ability to obtain prior to start date
● Current vehicle insurance.

Essential Functions
1. Provides consistently exceptional care at all times.
2. Performs initial and ongoing physical therapy evaluations according to APTA standards of practice as well as state standards.
3. Assesses, plans, implements and evaluates care in collaboration with the multidisciplinary care team, in accordance with CMS guidelines for Home Health and Hospice
4. Develops Plan of Care for each patient based on initial and ongoing evaluations including short and long term goals, collaboration demands of other staff members, and required patient and caregiver involvement.
5. Implements Plan of Care on a consistent schedule and adjusts this schedule and plan according to patient progress.
6. Documents patient progress according to approved medical documentation and facility documentation procedures.
7. Evaluates mobility, strength and current level of patient function, while setting goals to improve in these areas that are scaled to patient abilities and realistically attainable.
8. Assesses pain levels on all patients:
a. Upon initial assessment and reassesses according to standards
b. Evaluates location, severity, intensity and quality of pain.
c. Uses the approved pain scale for measurement
d. Reevaluates pain level in response to intervention/ treatment
e. Reports abnormal results to appropriate clinical nursing staff
9. Assesses psychosocial needs of patients, expressing interest in their progress and comfort and educates them to the plan of treatment.
10. In collaboration with the multidisciplinary team, effectively educates patients, families in treatments/rehabilitation techniques. Participates in patient care conferences.
11. Recognizes unusual conditions outside of his/her expertise and seeks input of the interdisciplinary team.
12. Reports patient progress to patient, appropriate caregivers, facility staff and healthcare professionals involved in ongoing treatment/care of patients.
13. Develops a plan for patients following completion of their Plan of Care with appropriate referrals.
14. Assists other facility staff members as needed to provide care and create a clean, orderly and effective environment for rehabilitation.
15. Assures continuation of therapeutic plan following discharge by designing home exercise programs; instructing patients, families, and caregivers in home exercise programs; recommending and/or providing assistive equipment.
16. Documents patient care services by charting in patient and department records.
17. Maintains patient confidence and protects hospital operations by keeping information confidential.
18. Maintains a safe and clean working environment by complying with procedures, rules, and regulations.
19. Protects patients and employees by adhering to infection-control policies and protocols.
20. Ensures operation of equipment by completing preventive maintenance requirements, following manufacturer's instructions.
21. Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.
22. Develops physical therapy staff by providing information; developing and conducting in-service training programs.
23. Complies with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions.
