Dates needed- 11 / 5-11 / 8, 11 / 27-11 / 30, 12 / 31-1 / 6 and then variable going forward

Does Provider need to be Board Certified- yes
Coverage Type- intermittent
How many Patients will Locum see per day- 3-6
Is there rounding? Yes, on current patients
Avg patients they round on? 5-8
How many calls do they get while on call? Variable, 0-2
