Job Summary:
The Clinical team at Basecamp Recovery Center is looking for a Licensed Social Worker (LSW) to provide clinical care coordination, facilitate group sessions, and assist patients with discharge planning.
Core Responsibilities:
- Facilitate evening IOP Groups: Lead and guide evening Intensive Outpatient Program group sessions on a weekly basis, fostering a supportive and therapeutic environment for patients.
- Discharge Planning: Develop and implement comprehensive discharge plans for patients in the BRC program, ensuring a smooth transition to the next level of care and continued support.
- Collaborative Care: Work closely and effectively with a multidisciplinary team, including physicians, therapists, case managers, and other healthcare professionals, to provide integrated and holistic patient care.
- Clinical Compliance: Adhere to established clinical standards, policies, and procedures to maintain a high quality of care and ensure patient safety.
- Individualized Treatment: Conduct thorough assessments to understand each patient's unique needs and develop personalized treatment plans that address their specific challenges and goals.
- Crisis Intervention: Be prepared to respond effectively to crisis situations, providing immediate support and stabilization to patients in distress.
- Documentation: Maintain accurate and timely documentation of patient assessments, treatment plans, progress notes, and other relevant clinical information.
- Professional Development: Participate in ongoing training and professional development activities to stay current with best practices in behavioral healthcare and enhance your clinical skills.
Job Duties:
- Document services performed, including progress notes and group therapy sessions.
- Utilize technical tools such as Google Docs, conduct internet searches for resources, and participate in Zoom communications.
- Perform intake assessments, crisis de-escalation, and other clinical coverage as needed.
- Lead or assist with group therapy sessions beyond IOP if required.
- Maintain accurate and timely documentation of patient progress and care.
Education and Experience:
- Active LSW license in the state of Ohio (required).
- Experience facilitating group therapy (IOP or similar) is preferred.
- Proficiency with Google Workspace and virtual platforms such as Zoom.
- Strong documentation, organizational, and time-management skills.
Schedule:
Monday/Wednesday/Friday: 8:30 AM – 5:00 PM
Tuesday/Thursday: 11:30 AM – 8:00 PM
